This game, called Whac-a-Critter on the box and Mallet Legend on the title screen, is an unlicensed action game for the Sega Mega Drive developed by AV artisan and published by Realtec exclusively in the US in 1993. It came with an unlicensed controller specifically designed for this game that is so rare very little information about it is known to the point that some doubt its existence (though you can use regular controllers with it, the controls with those will need figuring out).
The game is a Whac-A-Mole clone — simply hit the button on the controller corresponding to the hole where a critter pops up before it disappears. After hitting enough, you get a much harder "boss battle" before moving on to the next round.
