Story
Vietnam War veteran John Rambo returns to Vietnam to rescue lost POWs.
Gameplay
The game is an overhead run-and-gun played as Rambo. The objective of the game is to fight through the enemy's defenses, reach the gate at the end of each stage, and destroy the gate while fending off enemy reinforcements. The gate starts flashing in different colors when it is near destruction. Along the way, Rambo can destroy camps to free POWs and obtain bonus items. A second player can join in anytime during the first two stages by pressing , playing as Rambo's partner Zane (a character created for the game who wears a yellow headband).
Rambo is moved in any direction with the D-Pad. He is armed with an M-60 machine gun, which he fires with . The machine gun has unlimited ammunition (but only two shots can be out at a time). He also has a limited supply of explosive arrows that he can fire with , which are used to take out tougher obstacles such as tanks and gates. These arrows travel a set distance before exploding and can fly over targets. Rambo can only shoot in five directions (left, up, right, or diagonally up-left or up-right) because he faces forwards while moving backwards.
Rambo starts the game with three lives and gains an extra life for completing each stage. He loses a life when he is hit by an enemy. He starts each life with a supply of five explosive arrows and gains two more with every item collected, no matter which item it is (though the life and arrow counters max out at 9, players can actually have more than that). Power-up effects are lost when a player loses a life. The game for a player if that player runs out of lives.
Items
Items are obtained by killing a specific number of enemy soldiers or as bonuses from the prisoners when they are freed from the huts located throughout the first four stages (which can only be destroyed with explosive arrows). All items give Rambo two more explosive arrows when collected, in addition to their other effects.
